Director of Nursing at Heaton Woods

We are looking for a compassionate RN to be our Director of Nursing at Heaton Woods Residence in Montpelier. The Director of Nursing will provide oversight to our clinical team for the medication delegation and health monitoring of our residents. They will coordinate resident care with third-party providers and communicate with families of residents on current conditions as needed. 


The ideal candidate will possess a strong clinical background and have a working knowledge of long-term care regulation. The schedule would primarily consist of a Monday – Friday work week, with weekends and nights as needed and rotational on-call availability. Responsibilities

  • Is a good listener for residents, families, and team members and able to resolve or troubleshoot any issue in a positive, compassionate manner.
  • Delegates tasks to clinical team, oversees regular training, provides oversight to the team in the distribution of medication, health monitoring, and assistance with health-related tasks as allowed by regulation.
  • Responsible for ensuring safe clinical staffing numbers at residence at all times, working with the Clinical Scheduler to do so. Is responsible for interviewing candidates, along with the Director of Human Resources, and together making hiring decisions of team members while staying within budgetary guidelines established for the Residence.
  • Conducts or reviews/approves resident assessments; develops or reviews/approves resident service plans. Ensures up-to-date tiering status for all residents. Communicates updates and changes with resident and/or responsible parties through scheduled care plan meetings and situational updates.
  • Support the Administrator as needed for all Admissions
  • Provides coordination with Resident’s physicians and other health care providers
  • Manages coordination with the House pharmacy and such other outside pharmacies as may be needed.
  • Coordinates outcome of physician orders.
  • Reviews all therapeutic diets and food allergies with dietary staff as needed to assure nutritional standards are met and are consistent with physician orders.
  • Monitors the status of resident health conditions and initiates actions in response to changing needs; conducts re-assessments of resident health needs as indicated.
  • Implements assistive therapy as necessary to maintain or improve residents’ functional status, with consultation from a licensed professional as needed.
  • Provides health counseling/education to residents as needed.
  • Supports the Administrator and Director of Human Resources in resolving resident/family clinical dissatisfaction that may arise by investigating concerns, recommending changes in service policies and procedures with Administrator and Director of Nursing Services
  • Acts as the nurse on call when required.
  • Ensures nurses are on call when unavailable
  • Performs all other duties as assigned.

Education and Qualifications

  • Must have a current RN license in the State of Vermont and be a graduate from an accredited school of professional nursing.
  • Will be required to have their RN license on the facility.
  • Must have at least two years prior RN experience working with the elderly in either a residential/facility setting or in home-based care.
  • Must be at least 21 years of age. Must have a demonstrated ability to effectively communicate verbally and in writing.
  • Must work well with others, have effective training and supervisory skills, and be able to identify and address problems effectively.
  • Must be able to lift, carry, push and pull 40 pounds. Must be able to move about the facility quickly and be able to physically assist residents.
